Interested in becoming a provider for a family member?

Family is the closest natural support individuals can have. Family providers assist the members and/or their parents and guardians with an additional level of trust and understanding that can be difficult to build. To become a family provider, you will need to meet the minimum requirements. Please contact the office for more information on how to meet the minimum requirements.

Casa de Rosa has two open positions that require the following minimum qualifications. For more details on the open positions please click on each position.

Minimum Qualifications:

  • Be at least 18 years old
  • High School Graduate
  • Provide three professional references
  • CPR/First Aid required or ability to obtain
  • Currently hold or able to obtain Article 9 Certification
  • Currently hold or able to obtain Prevention and Support
  • Currently hold or able to obtain a clean fingerprint clearance card
  • Reliable transportation (This is a must)
  • AZ driver license, car registration, and car insurance
  • Successful completion of agency required training
  • Ability to lift a minimum of 50 pounds
I.H.S. Direct Support Professional

In-Home Support- Direct Support Professionals provide one on one services to members. These services include attendance care, habilitation, home keeping, and respite services. This position is flexible in hours and can include early mornings, evenings, and weekends. Please call the office for more information.

  • Classification: Hourly
  • Hours: Flexible schedule, including evenings and weekends
  • Rate: Depending on the service $15- $15.50
DTA Direct Support Professional

DTA Direct Support Professionals assist the DTA manager in completing the daily functions of the Day Treatment of Adults program. DTA DSPs may work one on one or up to a one-to-five ratio depending on each individual member’s needs. For more information, please call the office.

  • Classification: Hourly
  • Hours: 40 hours/ week, M-F, 8-5 PM
  • Rate: Depending on the service $15- $15.50
